Social Worker Skilled Nursing Facility Full-Time! APPLY TODAY!

Our long-term care Nursing Home Facility is currently recruiting a dedicated and experienced Social Worker.

As a Social Worker, provide medically related social services that assist the residents to attain or maintain the highest practicable physical, mental, and social well-being. 

Social Worker Director Key Duties

• Conducts pre-admission screening of individuals seeking admission to the facility, including persons with mental illness according to facility admission criteria.
• Completes and updates social services assessments for residents.
• Function as Care Plan Coordinator as assigned.
• Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care to protect and promote residents’ rights.
• Records progress notes in the clinical record. Observations of resident’s behavior and interventions
• Perform other job-related duties as they become required.

Social Services Director Qualifications

• Licensed Social Worker preferred.
• Two plus years experience with Seniors in healthcare setting. Skilled long-term care is preferred.
