Clinical Pharmacist at Providence Health and Services summary:

The Clinical Pharmacist is responsible for delivering individualized, cost-effective pharmaceutical care and pharmacy operational services in collaboration with healthcare teams to optimize patient medication outcomes. The role requires a PharmD or Bachelor of Pharmacy degree, a Washington Pharmacist License, and preferably acute care hospital experience or residency completion. Description

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Performs all duties in a manner, which promotes team concept and reflects the Sisters of Providence mission and philosophy.

Required Qualifications:

  • PharmD Doctorate of Pharmacy Or
  • Bachelor's Degree Bachelor of Pharmacy
  • Upon hire: Washington Pharmacist License
  • When eligible and where applicable, obtain a Board of Pharmacy Preceptor license
  • Where applicable Basic Life Support-Provider (BLS)

Preferred Qualifications:

  • Acute care hospital pharmacy experience, 1 year or equivalent experience
  • Completion of ASHP-accredited PGY1 residency
