Job Description

Job Summary Mission Community Hospital is looking for a detail-oriented Clinical Pharmacist to provide independent patient care activities, and to ensure the safe and effective use of medications. The Clinical Pharmacist will utilize their pharmaceutical knowledge to counsel patients and provide sound direction on use of available therapies. Responsibilities

• Review and verify accuracy of medication orders and medication history, making any necessary adjustments.
• Perform accurate clinical assessments of medication orders.
• Ensure safety of and responsible use of medications through medication reviews and monitoring, providing education and consultation.
• Work with other medical professionals to optimize therapy and provide quality medical care.
• Provide patient and provider education on medication use, side effects, drug therapy, and indications of medications.
• Collaborate with the hospital’s pharmacological, nursing, and medical staff to ensure patients receive the best possible medication use.

Requirements

• Graduate from an accredited pharmacy college
• Licensed as a Clinical Pharmacist in the state of California
• Get Healthcare Providers Certification (HCP) Certificate
• Minimum of 2 years experience working as a clinical pharmacy practitioner
• Must possess excellent communication and interpersonal skills
• Knowledge of current pharmacy practice and standards
